This form is for Crawfish Aquatics Swim Team & Adult Fitness Swim (Crawfish Aquatics Bluebonnet & Lafayette)

To withdraw from the program for the upcoming month, written notification must be given by the 25th of the month prior to be withdrawn from the billing system. Any refunds issued will include a $15 processing fee.

Upon submission you will receive an automated email copy of your form submission. Please keep for your records.

Notice

I understand that my membership will be cancelled as of the requested month. If member is entered in a non-Crawfish Aquatics swim meet with an entry deadline that has passed, there may be financial responsibility for meet entry fees regardless of participation. 

Withdrawal forms must be received by the 25th of the final month of team participation to be removed for the upcoming month. I understand that I will be billed for the next billing cycle if this notice is not received by the 25th of the member's final month, as per the registration agreement.  Any refunds issued will be less a $15 processing fee.
